CCUSNA collects the personal information contained in this Membership Form. We retain this information in electronic and hard copy form in our Membership Register as we are required to do under the Associations Incorporations Act 2015 (WA) (Act). The Membership Register is reviewed and updated at least annually and if you cease to become a Member of CCUSNA we will use all reasonable endeavours to remove your information from the current Membership Register within a reasonable timeframe. We will provide you with a copy of the personal information we hold about you as soon as possible upon your request. CCUSNA will only use your personal information in connection with the objects of the Association (as set out in the Rules) and will not share or sell your personal information to third parties for other purposes. Any information you provide to us will be treated as confidential except as required by Law. Under the Act, we are required to provide a copy of the Membership Register (which contains your personal information) to other Members of CCUSNA upon request. In addition to the matters above, Members of CCUSNA who access the Membership Register may not disclose the personal information contained in the Membership Register to, or use that information for the benefit of, third parties. By applying for Membership to CCUSNA you agree to this restriction on your use of personal information that you may obtain by reason of your membership. As a member of CCUSNA, you agree to adhere to the CCUS NA privacy policy.
